Pay Procedures:
- Paid on a weekly basis.
- You will receive your first paycheck on your second Friday of employment.
- Direct deposit is available in a banking institution(s) of your choice. Money is guaranteed to be in your account(s) first thing Friday morning. You will still receive a pay stub each week.
Vacation:
- You will be paid a bonus in lieu of vacation (as follows):
- On your first year anniversary you will receive a bonus check for 40 hrs. (Straight time pay.)
- On your second year anniversary you will receive a bonus check for 60 hrs. (Straight time pay.)
- On your third and subsequent anniversary you will receive a bonus check for 80 hrs. (Straight time pay.)
- Any days off will be unpaid.
- If you have been rehired and your absence was less than six (6) months, you will retain your original seniority date for vacation pay purposes.
Holiday Pay:
- Advancement recognizes 6 holidays. You are eligible for holiday pay after 90 days of employment. Former employees with less than six (6) months absence will be eligible for holidays immediately.

- Christmas Day
401(k):
- Eligible the 1st day of the month following 90 days of employment. Depending on your actual start date, it could be anywhere from 90 to 120 days before you are able to contribute to the plan.
- You may contribute from 1-20% of weekly compensation up to the maximum set by the IRS.
- Advancement does not match or contribute to the 401(k).
Anthem Blue Cross Blue Shield Lumenos Health Savings
Account Plan:
- Eligible the 1st day of month following 90 days of employment. Depending on your actual start date, it could be anywhere from 90 to 120 days before you are covered.
- This plan is good throughout the United States.
- Health Rewards Program allows you to earn Healthy Reward incentive dollars redeemable for gift cards.
- Deductibles are $2,000.00 per member and $4,000.00 per family, per year.
- Office visits are covered 100% after deductibles have been met.

Prescription:
- Receive Anthem’s discounted rates by visiting in-network pharmacies.
- Mail order prescriptions are available. You have the ability to buy in volume to potentially save money.
Anthem Dental PPO:
- Preventative services are covered at 100% in-network (Cleanings and Exams).
Anthem Blue Vision PPO:
- Receive Anthem’s discounted rates by visiting an Anthem Blue Vision provider (provider directory found at: http://www.anthem.com or by calling Anthem Blue Vision (Customer Service at 1- 800-828-3677).
- $10.00 co-payment for vision examinations, once every 12 months from an Anthem Vision Network Provider.
- $20.00 co-payment for frames and lenses, once every 24 months from an Anthem Vision Network Provider.

Group Life and Disability:
(Offered by MetLife Metropolitan Life Insurance Company)
- Effective date is the first of the month following 90 days of employment. Depending on your actual start date, it could be anywhere from 90 to 120 days before you are covered. Life and disability coverage is not contingent on a medical exam. However, you have only one chance to enroll for this guarantee issue period.
Optional Life: (for employee, spouse, and children) All premium(s) based on age of employee and amount of coverage chosen.
Short term disability: Benefits can be paid up to 60% of your basic weekly salary, up to $1000 maximum. Weekly short term disability benefits start on the 15th day of sickness or accidental injury and are paid up to 13 weeks.
Bereavement:
Available after 3 months of employment: One (1) day pay (equivalent to 8 hours straight time pay) with verification of death of certain
family members.
Jury Duty:
Advancement will pay the difference between the amount you receive for serving on jury duty (daily fee plus mileage) and what you would have made if you worked eight (8) hours of straight time. A receipt from the appropriate municipality is required.
$$$ Refer-A-Friend $$$:
Advancement pays referral bonuses for referring candidates that Advancement places on assignment. The bonuses range from $100.00 to $300.00 and are paid to you after the candidate completes 30 days on assignment with Advancement.
